Nappy rash on toddler - Ideas please!

I would really like your help and ideas...
Our daughter has been in cloth since she was born, and was potty trained at by months She's now 2 years and 2 months and still in night-time nappies and since being potty trained during the day has developed dreadful nappy rash. The rash was so bad that it became infected and has been on 2 lots of oral antibiotics and a topical cream. While we were using these we used disposable nappies (which I hate). The rash had completely cleared up, however within days of starting back in cloth the rash has started to re-appear.
We use Tots Bots Fluffles, boosted with either a cotton or bamboo booster and use a fleece liner next to her skin. We also use a fleece outer wrap. We washed the nappies at 60 degrees to rid them of any nasties, and we always use surcare non-bio powder.
I really want to keep using cloth, but am running out of ideas!
We moved onto fluffles at night time, partly as we thought they would be a better night time nappy that the bumbles that we were using, and partly due to the fact we needed the bumbles for our new arrival. Could the fluffles be causing the rash? My understanding is that the fluffle (microfibre nappy) is less likely to cause a reaction than cotton.
Any ideas would be greatly appreciated
