I have to agree, Summer is in mediums at 8lb, and they are a trim fit, definately go for the large!

Tanya

I use mainly itty bittys with Anais, have used mediums since she was 5 months and am still on the tightest setting..though not crossed over nd its still a bit loose on her tummy, the rise is also good on anais, still quite high on the back and just below belly button, but if i remeber right Bella is long bodied, so you might need a large.....the website says if your bubs is 20 pounds and you like a higher rise go for large.
I think itty bittys are really reliable, i dont use a snap in booster and it still can go hours and hours!!! Really slim fitting on, i will take a pic of anais wearing one tommorow and put it on fb for you to see what its like on her.
xx
