what size does a medium cover? Do they have a waterproof layer in them?x

HI,
No liner. You could use an AIO extra insert I suppose, but these are basically thicker underpants for potty training. More absorbent than underpants for accidents. But not waterproof at all.

I'd say it fits an 18month to 2 1/2 year old. I could do some measurements tomorrow.
